Why anything gets printed outside the box
Presses and cutting equipment work to a tolerance, not to a mathematical certainty. Sheets shift a fraction as they feed. Paper expands and contracts with humidity between the print pass and the cutting pass. Blades wear. Add those together and the cut lands slightly away from where the drawing said it would, sometimes by a sixteenth of an inch or more on large sheets.
If the background color stopped exactly at the drawn edge, every sheet that shifted outward would show a thin white line of bare stock along one side. That line is instantly visible on a dark box and it looks like a defect, because it is one. Extending the color past the edge means the blade always lands inside printed area, and the excess is simply thrown away with the trim.
Three boundaries on every artwork file
Any packaging file has three concentric lines and they do different jobs. The outermost is the bleed boundary, where the artwork actually stops. The middle one is the cut or trim path, where the blade is aimed. The innermost is the safety or live area, and nothing you need to read should cross it. The gap between cut and safety absorbs the same tolerance in the other direction, so a cut that lands slightly inward does not shave the top off your legal copy.

Rigid wraps are the outlier worth flagging. The printed sheet does not stop at the box edge at all; it folds around the chipboard and continues onto the inside face. That turn-in allowance is far larger than a conventional bleed, and artwork built with a standard eighth of an inch will leave raw white paper visible along the inside lip of a finished lid.
Setting it up so prepress does not send it back
Extend the actual artwork, rather than adding a stroke or a colored rectangle behind it. If a photograph runs to the edge, the photograph itself needs enough pixels to cover the bleed area at full output resolution. Scaling an image up to cover the extra margin drops its effective resolution and the softness shows on the finished panel.
Export with bleed switched on and crop marks offset beyond the bleed boundary, so the marks do not print inside the trimmed piece. Keep the cut and crease paths on their own layer above the art, unlocked for prepress but never merged into the graphics. Those paths belong to the dieline, and the bleed boundary is measured from them.
Watch the panels that are not obvious. On a carton, the tuck flap, the dust flaps and the glue tab all have edges that trim, and background color has to run past each of them. Files arrive regularly with a perfect front panel and bare white flaps, which produces a box that looks finished from the front and unfinished the moment somebody opens it.
Where bleed and folds interact
Bleed only applies to edges the blade will cut. A fold line is not a cut, so artwork crosses it continuously rather than stopping and restarting. Where designers get caught is assuming a fold behaves like a trim: they build each panel as a separate artboard with its own bleed, then the panels get assembled with hairline gaps at every crease.
The reverse mistake matters too. A graphic that must line up across a fold will shift slightly, because scoring pulls the material as it bends. Continuous patterns cross folds well. A precisely aligned wordmark spanning two panels does not, and it is worth redesigning rather than fighting.
Checking it before the run
